This form must be completed yearly and includes ALL volunteers/mentors on school campuses during operating hours or at any school-sponsored function.
Information obtained is confidential and used only for the purpose of ensuring student safety. All Criminal History Authorizations are deactivated annually on June 30th and after this date volunteers must complete another Criminal History Authorization form for the coming year. Applicants are notified via email of their approval status within 3 to 5 business days.
